Why should I enroll in this program?

 

We created the American Diabetes Alliance to help people who are diabetic or at risk of becoming diabetic have a better quality of life.  We help you achieve this three different ways: Prevention, Control and Management.

 
  • Prevention of complications associated with diabetes

  • Control of diabetes through proper diet and exercise

  • Management of blood glucose levels

 

·   What does it cost to enroll in the program?

 
There is no cost to enroll in the plan. There are no monthly plan premiums after a Member enrolls. In fact, the Member has $0 out of pocket cost for Member benefits, which may even include diabetic supplies. There are no hidden fees or cost with the program through the American Diabetes Alliance.
 

What are the benefits of enrolling in this program?

 
  • No Plan Premiums  There are no plan premiums for members. There is no hidden cost to join or to become a member. 
  • No Up-Front Cost for Supplies  There is no up-front cost for diabetic supplies for our members in this program.
  • Diabetic Kit  The Diabetic Kit each Member receives includes a blood glucose testing monitor, blood glucose test strips, lancet device and lancets, and glucose control solution.  The same supplies are available for people with diabetes whether or not they use insulin.
  • No Cost for Shipping  There are no costs for shipping diabetic supplies for members.
  • No Deductibles  There are no plan deductibles to meet for members
  • No Co-Insurance  There are no plan co-insurances to satisfy for members.
  • No Claim Forms  There are no claim forms to fill out for members. No more complicated claim forms to fill out!
  •  No Out of Pocket Cost  There is no out of pocket cost for diabetic services for members.
  • Care Coordinator Service  Our Care Coordinators are available to you as part of being a member.
    Our Care Coordinators contact each Member to access their current diabetic needs. Our Care Coordinators help with a wide range of diabetic issues that may affect a Members overall well-being. By identifying any changes Members may face, our Care Coordinators can offer solutions that address the Members total diabetic needs.  Our associates are one of the only companies of its kind to have Care Coordinators as part of the services offered to Members at no cost.
  • 24-hour Nurse Hotline  Our 24-hour Hotline Nurses are an immediate, reliable and caring source of health information and available to you as part of being a member. This 24-Hour Nurse Hotline service is for Members who otherwise could not receive the answers to non-emergency health related questions or inquiries when the Members primary care physician is unavailable or cannot be reached.
 
How do I qualify to enroll in this free and exciting program?
 
The plan is available to individuals at any age, in every state and every county who, by doctors orders, are required to check their blood glucose level and have a billable insurance (private pay, employer paid, Medicare or Medicaid). Individuals who have Medicare Advantage Plans are also eligible to enroll.
